📋 Tanggung Jawab Pekerjaan

Assess and evaluate the social, emotional, and psychological needs of individuals and families in the healthcare setting.,Develop and implement case management plans to address the identified needs and improve the overall well-being of clients.,Provide individual and group counseling, support, and advocacy services to clients.,Collaborate with healthcare professionals, community organizations, and other stakeholders to ensure comprehensive and coordinated care.,Educate and empower clients and their families on various health and social issues, promoting self-advocacy and independence.,Monitor and evaluate the progress of clients, making necessary adjustments to the case management plans as required.,Maintain accurate and confidential records of client interactions and progress, in compliance with relevant laws and regulations.

🎓 Kualifikasi & Syarat

Bachelor's degree in Social Work, Psychology, or a related field.,Minimum of 2 years of experience in social work, preferably in the healthcare sector.,Strong knowledge of social work theories, practices, and ethical guidelines.,Excellent communication, interpersonal, and counseling skills.,Ability to work independently and as part of a team in a fast-paced environment.,Familiarity with Indonesian culture, language, and healthcare system is a plus.,Valid certification or license as a Social Worker is preferred.
